The following forms are for those interested in the SMP program for those whose congregation has committed to call them when ordained.
Reporting Forms - Due August 1 each year:
This form is to be completed by the SMP pastor (you can type in the form) then print, sign, collect your Supervising pastors signature, then scan and email back to Martha Wohlfeil. If you do not have scanning capabilities then please mail the form to the district office.
